By signing this form, I authorize A&A Appliance Leasing and its authorized representatives to obtain and use a key or access code to enter my apartment/unit for the sole purpose of delivering, installing, servicing, or retrieving leased appliances.

I understand and agree that:

• This access is limited strictly to the services provided by A&A Appliance Leasing.

• All efforts will be made to respect the privacy and property of the premises during the visit.

• A&A Appliance Leasing is not liable for any personal items or damages not related to the appliance services provided.

• I may revoke this permission at any time by submitting a written notice to A&A Appliance Leasing.
